The New York State Education Department (“SED”) Office of the Professions (“OP”) is alerting everyone to a vishing scam that has been brought to our attention. Phishing—or “vishing”—scams impersonate SED employees or websites attempting to collect licensure and personal information from the licensee. If you receive an inquiry which you believe to be suspicious do not provide any information. To verify if the inquiry was from OP, contact us directly. You may report any suspicious communication received to the Federal Trade Commission.

You must submit the following documents to our office:

Upon filing with the New York State Department of State, the filer is required by law to submit to the Education Department:

  • A Certified Copy of the Certificate of Incorporation
     
    *Please note that an entity will not be listed on the New York State Education Department website until the certified copy is received.

    and
     
  • A $20.00 filing fee (check or money order made payable to the New York State Education Department).
     
    Note: A certified copy may be requested and obtained from the Department of State at the time of filing.
