The New York State Education Department (“SED”) Office of the Professions (“OP”) is alerting everyone to a vishing scam that has been brought to our attention. Phishing—or “vishing”—scams impersonate SED employees or websites attempting to collect licensure and personal information from the licensee. If you receive an inquiry which you believe to be suspicious do not provide any information. To verify if the inquiry was from OP, contact us directly. You may report any suspicious communication received to the Federal Trade Commission.

Miscellaneous Information for Professional Practice Entites

Address Updates

To change a professional practice entities' address on file with the New York State Department, submit a letter on company letterhead which includes the following:

  1. Name of the professional practice entity;
  2. PSC Number;
  3. Old address on file for the professional practice entity;  
  4. New address for the professional practice entity;
  5. signature of at least one member of the professional practice entity; and
  6. date.

Note: This should be on company letterhead and must be emailed to the Professional Corporations Unit at opcorp@nysed.gov.
