License Application Status

Find the status of your license application by checking our online license verifications; your name will be added immediately when your license number is issued.

We cannot provide the status of a licensure application by phone. After you have submitted all your documentation, please allow 6 weeks before submitting a Contact Us Form to request a status update. Contacting us earlier may extend processing times for applications. During busy times, this time frame may increase. We thank you for your patience and cooperation.

Please provide your name, the last 4 digits of your social security number, date of birth, and the name of the profession in any correspondence.

Professional Misconduct and Discipline

Professional misconduct by licensed physicians, physician assistants, and specialist assistants is investigated and prosecuted by the Office of Professional Medical Conduct in the New York State Department of Health. Complaints of misconduct in these three professions should be made to the New York State Department of Health, Office of Professional Medical Conduct, Riverview Center, 150 Broadway, Suite 355, Albany, NY 12204-2719. You may call that office at 1-800-663-6114 (Monday through Friday 9:00 am to 5:00 pm eastern) or e-mail the office at opmc@health.ny.gov.
