Instructions

• Complete Endorsement Application (OF102 - 9 KB)
• Complete M/W Information Form (OF111 - 8 KB)
• Check or Money Order for $170 payable to the New York State Education Department

In addition, submit the following:

1. Lease or deed for the premises made out to the registered name of the corporation.
2. Certificate of Occupancy for the premises.
3. Photo of outside signs on the building.
4. Photo of front exterior sign showing the registered name of the corporation on the exterior.
5. FDA registration for new location.
6. FDA inspection for new location.

You will be notified by mail regarding the inspection at the new location
