Important Note for Individuals Who Have Already Filed an Application as a Certified Histotechnician: Changes to the Education Law that took effect on January 17, 2023 allows individuals to apply for certification as a histotechnician based on meeting new criteria described in the requirements section. As a result, individuals who have already submitted an application and fee with the Department may now be eligible to meet the new criteria. If you applied as a certified histotechnician previously but did not meet the requirements in effect at the time, you may use the Form 1A to apply to be considered under the new criteria. There is no additional fee required for this option. If you have previously withdrawn your application for certification as a histotechnician and received a partial refund, you cannot use this option.
Online Form 1 - Application for Certification
All applicants for licensure must initially submit Form 1 along with the $245 licensure and first registration fee. You must answer all questions and provide all information requested unless otherwise indicated. Failure to accurately complete all required parts of the application will delay its review. Additional forms below are required based on the licensure requirements of the profession. Do not use Form 1 to renew your existing license.

Online Form 1A - Application for Certification
Only use this form if you have already filed an application and fee for certification as a certified histological technician with the Department. Submit the Histotechnician application for certification (Form 1A). The Department will review your application and reach out to you directly if any additional information is needed.

Form 2 - Certification of Professional Education
This form must be submitted directly by the educational institution(s) you attended. The Office of the Professions will not accept this form if submitted by the applicant.
- Section I: Complete this section before sending the entire form to your school. Be sure to sign and date item 9.
- Section II: The Registrar must complete this section and return both pages of the form along with any required documents directly to the Office of the Professions at the address at the end of the form.
Form 3 - Verification of Other Professional Licensure/Certification
Complete this form if you hold, or ever held, a license or certificate to practice any profession* in any jurisdiction.
This form must be submitted directly by the licensing/certifying authority. The Office of the Professions will not accept this form if submitted by the applicant.
- Section I: Complete this section before sending the entire form to the licensing/certifying authority of each jurisdiction in which you are or have been licensed/certified. Be sure to sign and date item 10.
- Section II: The licensing/certifying authority must complete this section, sign, date and return both pages of the form directly to the Office of the Professions at the address at the end of the form.
* Profession is defined as one of the professional titles licensed under New York State Education Law.
NOTE: This form is to verify other professional licensure only and should NOT be used to verify New York City Certification of Qualification or any affiliations with professional associations or organizations. A Form 3 is not required for licenses/certificates issued by the New York State Education Department. Also, if the certification/verification documentation provided by a U.S. jurisdictions contains the same information requested in the Form 3, completion of a Form 3 for that jurisdiction is not necessary. For certification/verification documentation that does not contain the same information, a Form 3 will need to be completed for that jurisdiction.
Online Form 5 - Application for Limited Permit
All applicants for a limited permit must initially submit Form 5 along with the $50 limited permit fee. You must also submit a Form 5CS - Certification of Supervisor for Limited Permit along with your application.

You will need the Application ID of your limited permit submission that was emailed to you and your date of birth. A new fee is not required for a limited permit issued as a result of a change in supervisor or setting.

All applicants for a provisional permit must initially submit form 5PP along with the with the $50 provisional permit fee at the same time or any time after your submit your application (form 1) and $345 licensure and first registration fee.
