The New York State Education Department (“SED”) Office of the Professions (“OP”) is alerting everyone to a vishing scam that has been brought to our attention. Phishing—or “vishing”—scams impersonate SED employees or websites attempting to collect licensure and personal information from the licensee. If you receive an inquiry which you believe to be suspicious do not provide any information. To verify if the inquiry was from OP, contact us directly. You may report any suspicious communication received to the Federal Trade Commission.

The pharmacist transferring the prescription is responsible for invalidating the original prescription on file as of the transfer date.  They must also document the following information, either in writing or electronically:

  • the name of the patient;
  • confirmation that a valid prescription has been transferred;
  • the name, address, and telephone number of the receiving pharmacy;
  • the name of the pharmacist receiving the prescription transfer information;
  • name of the pharmacist transferring the information; and
  • the date of the transfer.

The pharmacist receiving the transferred prescription is responsible for maintaining a complete and accurate record of the prescription transfer. The word “transfer” must clearly appear on the prescription, and the following information must be documented, either electronically or in writing:

  • the name of the patient;
  • confirmation that a valid prescription has been transferred;
  • the name, address, and telephone number of the pharmacy from which the prescription was transferred;
  • the name of the pharmacist receiving the prescription information;
  • the name of the pharmacist transferring the prescription information;
  • the date of the original prescription and, if applicable, most recent transfer; and
  • the original prescription number.
