The New York State Education Department (“SED”) Office of the Professions (“OP”) is alerting everyone to a vishing scam that has been brought to our attention. Phishing—or “vishing”—scams impersonate SED employees or websites attempting to collect licensure and personal information from the licensee. If you receive an inquiry which you believe to be suspicious do not provide any information. To verify if the inquiry was from OP, contact us directly. You may report any suspicious communication received to the Federal Trade Commission.

Form 2B - Certification of Professional Acupuncture Education

This form must be completed and submitted by the educational institution. The Office of the Professions will not accept it if submitted by the applicant.

  • Section I: Complete this section of the form before sending it to the school(s) where you completed your professional acupuncture education.
  • Section II: The school Registrar must complete this section and return the form directly to the Office of the Professions at the mailing address at the end of the form.

The school must attach an official transcript or a notation of all courses you completed. The transcript must bear the original signature of the Registrar and original seal of the school. If you attended more than one school to fulfill the professional education requirement, copy this form and send a copy to each institution.
