Clinical Laboratory Technologist

You may apply through this pathway until September 1, 2013.

Use this method if you have 6 months (at least 720 clock hours) of experience between December 31, 2004 through December 31, 2007 as a clinical laboratory technologist and your experience and competence are attested to by a director of a clinical laboratory regulated by the Department of Health. 		September 1, 2013 Experience:
6 months (720 clock hours) between December 31, 2004 and December 31, 2007

Clinical director attests to both experience and competence.

Laboratory licensed by DOH		 Until Department takes final action on application.

Clinical Laboratory Technician

Method 1A
You may apply through this pathway until September 1, 2013.

Use this method if you have 6 months (at least 720 clock hours) of experience from December 31, 2004 through December 31, 2007 as a clinical laboratory technician and your experience and competence are attested to by a director of a clinical laboratory regulated by the Department of Health.		 September 1, 2013 Experience:
Six months (720 clock hours) from December 31, 2004 through December 31, 2007.

Clinical director attests to both experience and competence.		 Until Department takes final action on application.

Cytotechnologist

Method Application Deadline Will Have Completed Requirements By: Can Continue to Work Until:
Methods 1, 1A and 2 were only available until January 1, 2009.

Certified Histological Technician

Method 1A
Use this method if you have 6 months of experience (at least 720 clock hours) and competence as a certified histological technician attested to by a director of a clinical laboratory regulated by the Department of Health from December 31, 2004 through December 31, 2007.		 September 1, 2013 Experience:
Six Months (720 clock hours) between
December 31, 2004 and December 31, 2007 		Until Department takes final action on application.
