License Options

Certified Histological Technician

New York State law allows two options for the practice of Clinical Laboratory Technology as a Certified Histological Technician.

Full License

Limited License

Limited licenses expired on August 31, 2018 and cannot be renewed. This option is no longer available.

Individuals who were issued a limited license before August 31, 2018 may apply for a provisional permit by submitting Form 5PP - Application for Provisional Permit.

Provisional Permit

On the recommendation of the board, the department may issue a provisional permit to practice as a clinical laboratory practitioner to an applicant who is employed in a clinical laboratory to enable the applicant to complete the education requirements and/or to pass the exam required for licensure as a certified histological technician. To be issued a provisional permit, the applicant must meet the requirements outlined in Section 8608(2)(b) of Education Law. Once issued, the provisional permit holder must practice under the general supervision of the director of a clinical laboratory, as determined by the department. The provisional permit is valid for a period of not more than one year, and may be renewed, at the discretion of the department, for one additional year.

Provisional permit applicants who wish to continue to practice after the permit expires should also apply for a full license as a certified histological technician by submitting an application and fee for a full license. To ensure that there is enough time for the application for a full license to be reviewed and approved within the one-year period when the provisional permit is valid, the application should be submitted at the time of, or close to the time when, the application for the provisional permit is submitted.
