Summary: Licensee was found guilty of having been convicted of Arson in the 2nd Degree and Assault in the 3rd Degree based on his starting a fire in a building inhabited by a woman who had been his girlfriend, under circumstances where the presence of persons in the building was a reasonable possibility and assault of his former girlfriend.

Summary: Licensee was found guilty of having been convicted of Vehicular Manslaughter in the 2nd Degree, Operating a Motor Vehicle while Under the Influence of Alcohol and Endangering the Welfare of a Child based on that she operated a vehicle in an intoxicated condition with her six-year old son as a passenger in the car and drove the car in a manner which resulted in striking and killing a boy on a bicycle.

Summary: Licensee was found guilty of having been convicted of Willful Violation of Health Laws based on her administration of methadone to a patient which was meant for a different patient while employed at a Health Care Center and that she did not report it or seek medical attention for the patient.

Summary: Licensee was found guilty of having been convicted of Falsifying Business records in the 2nd Degree based on that while she was chief accounting officer for a publicly traded corporation, she fraudulently made false and improper entries in the business records of that corporation regarding the dates that stock options were granted so that expenses for the corporation were understated and income was overstated in a 2002 financial statement submitted to the SEC.
